Terms and Conditions

We have a Service Provider agreement with certain insurance houses to deliver a worksite travelling service to their clients.
We offer the following services in designated areas:

1. Collection of blood samples.
2. Collection of urine samples
3. Short Medical Reports *

Should you wish to make use of this service the following should be taken note of:

Please note that this service will be delivered by our laboratories within a 20km radius of the service points listed below.

*Please note that until negotiations have been completed no Short Medical Reports (SMR) will be completed within the Cape Town Region.

The following details of your client must be provided:
 

1. Policy Number
2. Identification Number
3. Full name and surname of the life assured.
4. All contact details:   Full physical work address, telephone numbers, cell phone numbers
5. The tests that need to be performed.
6. Short medical report required or not.
7. Your name and contact telephone number

The laboratory

After having received the request, it will be task logged by the laboratory and assigned to a specific Nursing sister.

The sister will confirm the client’s availability telephonically prior to departure for the worksite. She will identify the client as mentioned per LOA Code of Conduct. The blood- and urine samples will be collected. When applicable, a Short Medical Report will be performed on site.

The blood samples will be taken to the laboratory. Results will be delivered electronically per EDI as before. The Short Medical Reports will be taken to the laboratory and faxed to the underwriting department. The Insurance Company will be billed electronically.

General notes

Please ensure that you still offer the option of personal pre-test counselling (PPTC) for HIV testing to your client. However, if the client elects to undergo PPTC, this will still have to be performed on an appointment basis at those laboratory depots that offer the service. As this a personal and confidential event, PPTC cannot be offered in the workplace.

• Offer the service to your client. The alternative options of the client going to a laboratory venesection depot, or the client’s own family doctor, are still available.
   
• If the client prefers to be visited at work (no home visits allowed), inform him/her that a Nursing sister will confirm a convenient time telephonically. The service will be available within normal office hours only (excl. weekends).
   
• Inform the client that he/she should have his/her S.A. ID book, RSA passport or valid SA card type driver’s licence available for the sister’s visit. Without such identification, the sister will refrain from collecting the required items.
   
• Provide the client with a completed laboratory request form. Inform the client to read the HIV testing information form.
   
• If a Short Medical Report is applicable, assist the client in completing the pages comprising of the client’s medical history. The client should have the laboratory request form, and the Short Medical Report when applicable, ready for the visit by the Nursing sister.
